Important things to know so you can get the most out of your Physical Therapy
Do I need a referral for PT?
No! in the state of Colorado, no referral is needed to see a Physical Therapist. You are always welcome to contact your primary care physician first, but it’s not necessary. If you are ready for PT, self schedule your initial evaluation or contact Team Summit Physio & Performance for assistance to see if you are good fit. Continue reading to learn more.
How to prepare for your initial evaluation
Once you have an initial evaluation scheduled, please follow the steps below to prepare for your appointment
Step 1: Register your account with PtEverywhere
When you schedule your initial evaluation, you will receive a confirmation email from from Summit Physio. In that email are simple instructions for setting up your PtEverywhere account, which is our patient portal. Please do that so you can move to step 2. If you don’t see a confirmation email in your inbox, please check your spam folder or contact Summit Physio & Performance for help.
Step 2: Fill out your new client forms
Once you set up your PtEverywhere account, log on and fill out the new patient intake documents. Please do this prior to the day of your appointment so your provider can best serve you.
What to expect in your initial evaluation
What happens in the initial evaluation?
During you initial evaluation, your physical therapist will map out your symptoms on a body chart, take a detailed medical history & conduct subjective exam. An individualized evaluation will be performed to determine the root cause of your symptoms or problems.
Will I receive treatment during the initial evaluation?
Your physical therapist will typically perform a trial treatment in order to start the healing process on the first day
What should I wear?
Please wear comfortable athletic clothes to your appointment. You need to be able to move your body and your PT needs access to your torso, arms and lower body. Ideal attire is athletic shorts and a sports bra for those who need it.
What next?
After your first appointment, you will have a better understanding of what may be contributing to your pain, mobility, problem, or performance issue. You will leave with a customized home exercise program through Medbridge. You will have the best results if you follow the plan your physical therapist gives you.
