Payment methods

We require you to have a credit card on file. You may use that card for payment, however, we also accept cash, check, HSA or FSA as well. Please continue reading for information on insurance.

Do we take insurance?

Summit Physio & Performance is an out of network Physical Therapy company that does not bill insurance

Here is why:

For Summit Physio & Performance to bill insurance:

  • We would be required to co-treat multiple patients at once. This means you would be treated with multiple other patients, often for an insufficient amount of time, depending on insurance requirements.

  • We would be denied many performance based treatments that our Doctors of Physical Therapy utilize, making it less likely to get the outcomes you need.

In our professional opinion, eliminating the insurance requirements allows us to tailor our 1 on 1 patient focused care & achieve the effective results we strive to provide.

Can you bill your insurance for reimbursement?

Generally YES! These FAQ’s will hopefully answer your billing questions to the best of our ability. When in doubt, contact your insurance company for assistance.

Where do I get my Superbill?

After each visit you will have access to you Superbill through your Payments tab in your PtEverywhere account.  This will allow you to easily submit proof of your appointment to your insurance for out-of-network reimbursement. 

How do I send my claim?

Every insurance company is different. You should be able to print a claim form from your insurance company’s website & send it in with the needed receipts & treatment codes. Please contact your PT directly for those codes.

How much will be reimbursed?

The amount of reimbursement or application towards your deductible is completely dependent on your insurance plan. You call your insurance company to inquire about what you can expect to receive. Ask for information on reimbursement for “out-of-network Physical Therapy” expenses sent in via self-claims.

What about Medicare?

You will likely not be able to submit for reimbursement with Medicare. If you are on Medicare and you decide to work with Summit Physio, please let your Physical Therapist know when you schedule your Initial Evaluation as you will need to fill out some extra paperwork.
